The LCH Community Health Center in Kennett Square, PA

We are dedicated to helping adults, teens, and children to become healthy and stay healthy.

Our team of primary care providers, chronic care specialists, nurses, behavioral health consultants, case workers, medical assistants, and interpreters work as a team to provided integrated, Person-Centered Care to the greater Kennett Square community in southern Chester County.

Insurance or no insurance, LCH will take care of you. We evaluate everyone to determine eligibility for discounted services. Our sliding scale fees are based on your income and family size. We don’t turn anyone away due to inability to pay.

LCH Health and Community Services Kennett Square is located on West Cypress Street, just outside of the Kennett Square Borough.

LCH will help you build an ongoing relationship with your primary care provider here in Kennett Square, so that we can quickly understand and easily address changes to your health. Our doctors and medical specialists see all ages, but for infants, we encourage you to schedule an appointment with our pediatric office in West Grove.

We emphasize preventive care, and educate our patients on how life experiences and influences at work and at home can affect overall health.

Through our integrated electronic health record system, LCH Athena Health, service providers can understand social and environmental influences firsthand on each patient. The LCH team has a full understanding of a patient’s situation, and works together to help move the patients forward to the next level of health, independence, and confidence.

Care Management

Care Management will help you to keep your health top of mind.

This program will give you ongoing encouragement and resources from all the different support services at LCH. We all get so busy, and sometimes our health suffers from day-to-day stress.

Care Management at LCH includes an extra set of free appointments with our team of health experts that will help you reach your lifestyle and treatment goals. Just say yes to the program during your visit, and LCH will take care of the rest!  We will contact you to coordinate your Care Management appointments for dates and times that best fit your schedule.

Commitments

  1. Free Telehealth appointments with a Behavioral Health Consultant (BHC) at least four times a year.
  2. Face-to-face visits with your Provider (and BHC during the same visit) at least four times a year.
  3. Making a commitment to your lifestyle goals.

The doctors and medical staff at LCH Kennett Square are dedicated to providing care for the whole community, regardless of insurance status or income level. We accept most private insurances, Medicaid, and Medicare, as well as patients without insurance. Our sliding fee scale is based on income level and family size, so health services are accessible, affordable, and fair for everyone. Regardless of insurance, we screen every patient to see if they qualify for our reduced fee services.

    My most important heath tip for patients is to choose the healthier option that you can every day. For example, use the stairs over the escalator when going to the second or third floor in a building.

    Why are you excited to work at LCH?

    I am excited to work at LCH because the mission of providing equal access to healthcare for all is a passion of mine. I am excited to form partnerships with patients and their families so we can work together to maintain and protect their health and well-being through evidence-based prevention and management measures.

    What is your most important health tip for patients?

    Be as active as you can because our muscles and joints like when we keep moving. If it is difficult to set aside time to be active, put on your favorite music and dance around your home. It can be a solo activity or group activity and it’s super fun. It is good for the mind, body, and spirit.
