Understanding Diabetes: A Primary Care Perspective

As a primary care provider, one of the most common—and impactful—chronic conditions I help patients manage is diabetes. Whether it’s type 1, type 2, or gestational diabetes, this condition affects how your body processes blood sugar (glucose), and it can influence nearly every aspect of your health. The good news is that with education, routine care, and support, diabetes can be effectively managed.

Diabetes is often called a “silent disease” because symptoms can go unnoticed for years. I’ve had patients come in for a routine visit and be surprised to learn they have prediabetes or type 2 diabetes. That’s why regular checkups and blood work are so important. Early detection allows us to take action before complications arise.

In primary care, our approach is to treat the whole person. When someone is diagnosed with diabetes, we look at their diet, activity levels, mental health, medication needs, and access to resources. Managing diabetes isn’t just about taking insulin or avoiding sugar—it’s about building a lifestyle that supports stable blood sugar levels and overall health.

Education is key. I spend time helping patients understand how their bodies respond to food, how to read nutrition labels, and how stress and sleep affect glucose levels. We also set small, realistic goals.

I often refer patients to other members of our care team, including behavioral health specialists and pharmacists. Diabetes management works best when it’s a team effort. And when patients feel supported and informed, they’re more likely to stick with their treatment plan.

One thing I always stress to my patients is this: you are not alone. Millions of people live healthy lives with diabetes, and there’s no shame in a diagnosis. What matters is what we do next—together.
